  - The bidder is not “blacklisted” or barred from bidding by the government or any of its agencies, offices, corporations, or Local Government Units (LGUs), including non-inclusion in the Consolidated Blacklisting Report issued by the Government of the Philippines.
  - The bidder is not “blacklisted” to World Bank or any financial institutions

Independent Practitioner

1. - Curriculum Vitae (CV) (instead of company profile)
  - Tax Identification ID or Number
  - Two (2) valid government issued IDs
  - Updated PRC License or any Professional License, if applicable

Scrap Buyer

1. - Transfer Certificate Tile (TCT) of Junk Yard –secured and with at least 1,000 sqm. For leased property, Contract of Lease and TCT of property being rented.
  - Working Capital to at least P500,000
  - Copy of OR/CR of owned or leased vehicles.
  - DENR-EMB permit to transport (for buyers that shall transport scrap materials containing toxic and hazardous waste) or Cert. of Non-coverage

Construction Services

1. - Cert. of Training on Basic Occupational Safety and Health and/or Construction Occupational Safety and Health (COSH)
  - PCAB License
  - List of Completed Projects with contract value Project Start date, completion date, Project duration and Certificate of Completion.
  - List of Ongoing Projects with contract value, Accomplishment To date, Project Start Date, expected completion date and duration.
  - DO 174 Certificate of Registration or Certificate of Exemption
  - DOLE Clearance -Certificate of No pending case
